CNA Requirements

You must be at least 16 years of age. (Applicants under the age of 18 require written permission from a parent or legal guardian in order to enroll.

Must present a valid Driver’s license or State ID.

Must present a Social Security card.

Must be able to read, write and understand English at 9th grade level.

Must be physically and mentally able to perform task / skills.

Must provide documentation of a Negative 2-step TB skin test, TB blood test or chest xray.

Test must be completed within 90 days of your last clinical day and results must be in prior to the first day of class.

Must provide proof of seasonal flu shot (October 1st - April 15th only). Documentation must be presented to the office before the first day of class.

Must provide medical documentation from a physician if you are pregnant with no medical or physical restrictions. You will not be allowed to attend class or clinical without this release form from your physician. This form must be up to date and be on file before the class starts.

All ADA documents and forms must be up to date and turned into the office before class starts.

Must be drug free and alcohol free while attending class and clinical.

Must be free of criminal convictions or anything that will prevent the student from being added to the Nurse Aide Registry.

Must be able to attend 100%of all class and clinical dates.

Must be on time and stay for the full class and/or clinical date.
